AUSTRALIA

- In QLD (ILLEGAL) must be 18+.

- In NSW it is (LEGAL) for a Young Adult aged no less than 16 to have a Tattoo Procedure carried out.

CONDITIONS: (Mum, Dad or Guardian) must be present atleast up until all paperwork, Online Forms, etc have been complete and written consent given/granted.

ID REQUIRED:

MUM or DAD: (BIRTH CERTIFICATE, CURRENT DRIVERS LICENCE, PASSPORT & MEDICARE CARD).

GUARDIAN: (BIRTH CERTIFICATE, CURRENT DRIVERS LICENCE, PASSPORT, MEDICARE CARD & DOCUMENTATION PROVING THAT THEY ARE THE LEGAL GUARDIAN SUCH AS - court documents, centrelink documents, etc).

This information is required as it goes with our Online Form.


IF PARENT/S CAN NOT ATTEND STUDIO

If parent/s cannot come to the studio for whatever reason/s to complete the Online Form and provide ID they can attach a clear photo of themself 'selfie' to the Online Form holding their Drivers licence or 18+ Card close to their face (example shown below) as well as complete all information relevant on the Online Form.
'accept the TATTOO SATISFACTION REPORT which is more explained below'.

NOTE: this must be a *clear photo so we can match up the face to the ID and be able to clearly read the lastname, date of birth, etc printed to card.
*If the photo is not clear it will not be accepted.

Once all is done above and accepted by studio you may attend with Uncle, Aunty, Brother, Sister, a support friend or
even just yourself on proviso the tattoo is only of what was allowed by parent/guardian as stated on the online form they completed before attendance.

THE TATTOO SATISFACTION REPORT

On the Online Form at the bottom there is a final stage where it requires you to input your satisfaction with the Tattoo.
NOTE: You can complete all information and submit ID on this form without the need to complete the satisfaction information from home,
However, after the tattoo is complete then in the studio you and your tattooist can submit the remaining information on a separate form.
